Actually, on second thought, I think the 2nd pic may not necessarily be bad design if the owners of the facility want to ensure nobody is doing drugs or smoking in the toilet.

Must be quite a jialat school or workplace if they have to resort to such measures to prevent people from smoking or taking drugs...
